On iPhones with Apple Intelligence turned on, BloodWork uses Apple's on-device language model to catch values the rules missed and to write a plain-language summary. It runs on the iPhone's own chip. Anything it suggests is marked so you can check it.
On other iPhones, BloodWork uses its own rule-based reader and summaries. Both work fully offline.
